To request any type of tickets for any Bay Street productions, please download this form or pick one up in the Administrative office and fill it out completely.

Once the form is completey filled-out, please submit them to John Sullivan, Associate Producer / Company Manager by either putting in his mailbox in the Administrative office, or emailing the attachment to: (john@baystreet.org).  Please put your name and "Ticket Request" in the subject line.

Do not hand these forms directly to the box office.  They must be processed and approved before being sent to the box office.
